Customer Service Claims If you have any customer service claims,
please submit a Customer Service Claim (CSC) Submission Form. This form will provide
us with the information necessary to quickly process your claim. Customer
Service Claim Submission Form Instructions:Select
the Customer Service Claim Submission Form button at the bottom of this page and
complete the following information.

After
completing the Customer Service Claim (CSC) Submission Form, click on the SUBMIT
FORM button located at the bottom of the form. If your claim
is accepted, Corken will return (by email) the Customer Service Claim (CSC) Submission
Form with a CSC number. If your claim is rejected, Corken will supply a reason
for rejection. NOTE: Returned parts or equipment must
have the CSC number written on the outside of the box or attached to the equipment.
If you are sending back small parts, write the CSC number on the outside of the
box and use the Customer Service Claim (CSC) Submission Form as a packing slip
and place inside the box. Please include the part numbers and quantities on the
(CSC) Form.
